Onsite Inspection & Accident Investigation
We offer comprehensive onsite inspection and accident/failure investigation services, providing in-person evaluation, thorough documentation, and expert assistance in root cause analysis and corrective action planning.
These onsite assessments are particularly suitable for incidents involving major system and equipment failures, structural integrity issues, or scenarios where the failed component for analysis cannot be feasibly relocated.
What We Do Onsite
Visual Inspection and Documentation
Inspection and documentation of the existing condition of the component or through detailed photographic records and technical notes, capturing fracture surfaces, deformation, corrosion, wear, and other pertinent features.
Preliminary Failure Analysis
An initial assessment is conducted to characterize the failure mode, such as overload, fatigue, or corrosion, and to identify areas requiring additional investigative testing.
Sampling and Evidence Management
When the removal of failed or associated components is necessary for further analysis, procedures are implemented to ensure critical attributes are preserved. Comprehensive records of sample collection and handling are maintained as required.
Coordination
Direct collaboration is facilitated with client teams, contractors, and onsite inspectors to ensure joint understanding of inspection objectives and subsequent actions.
Follow-Up Testing (If Needed)
If circumstances necessitate analyses beyond visual inspection, specimens may be transported to our laboratory for further examination.
A test protocol will be developed to specify the recommended testing and analyses to determine root cause of the failure, such as but not limited to:
Low and high magnification fractographic examination
Mechanical testing (tensile, impact, hardness, bend)
Chemical analysis (OES)
Metallography (microstructure, fracture evaluation, etc.)
